Auto-Debit Form

Auto-Debit Payment Authorization Form

When submitting your Auto-Debit Payment Authorization Form, please attach a void check from your bank, savings account or credit union checking account.

Then mail (we will not accept fax copies of your Auto-Debit Payment Authorization Form) both the form and the void check to us. If you prefer to pay from your savings account, please obtain the ABA number and your account number directly from your financial institution.

Please note, it will take a few weeks to set up your auto-debit. To assure your payments are made on time, you will need to submit your monthly payments in the standard format until you are notified of your auto-debit begin date.

Federal PLUS Master Promissory Note (MPN)

Before requesting this application, click HERE to determine if the school your son or daughter is attending is a FFELP or Direct Loan college. If he or she is attending a FFELP college, you will be eligible to use this application. (If he or she is attending a Direct Loan school, you will need to get an application from your college.)

In addition, when completing this application, you will need to select the lender you wish to use for your student loans. Click HERE to see a list of Iowa Student Loan's lenders. We can include your lender code and name on the application we send you if you indicate that code here:
